Pioneering Transition Programs; The Establishment of Programs that Span the Ages Served by Child and Adult Mental Health
The University of Massachusetts Medical School produced this report for the American Institutes for Research with the purpose of identifying and describing the establishment of pioneering transition programs for individuals with serious mental health conditions.  This report refers to programs that serve youth continuously across the transition age, without disruption due to age changes, and offers general guidelines suggested that other programs can learn from.
